OSHA Training Boot Camp

Cancellation/ Rescheduling Policy

“U”nique Dental Services-OSHA Training Boot Camp reserves the dates you schedule for your personal training thereby preventing “U”nique Services from scheduling training for other customers on those date(s).

Rescheduling is allowed on a one-time basis only, when rescheduling is within 60 days of original scheduled training. No further rescheduling permitted for that annual year.

Rescheduling an onsite training class is permitted with 30 days advance notice requests must be received in writing by email. The client is responsible for ensuring that “U”nique Dental Services receives the written request. Unique   Dental services will reply with a written email acknowledgement.

Once the client pays the deposit and the client chooses to reschedule with less than 30 days advance notice, a $150 reschedule fee is charged. The rescheduled onsite class must take place within 12 months of the originally scheduled onsite class. If the onsite class does not take place within 12 months of the original onsite date, all deposits, fees and remaining balance will be forfeited.

OSHA training is required annually within 365 days of previous training.  OSHA training Boot Camp & “U”nique Dental Services are not responsible for clients that have not scheduled in appropriate time frame of any OSHA inspection or reprimand.

If the client cancels onsite training at any time, the deposit will not be reimbursed. If there are non-refundable expenses incurred by “U”nique Dental Services for OSHA Training Boot Camp on the client's behalf, the client is obligated to pay the additional cost. Any remaining balance of the training fees will be refunded to the client.

 U”nique Dental Services reserves the right to cancel training at any time. If “U”nique Dental Services cancels the onsite training due to weather or unforeseen circumstances beyond the control of “U”nique Dental Services – OSHA Training Boot Camp, the client is entitled to a full refund of the course fee, or your course fee can be credited toward a future onsite training, which would be rescheduled at your convenience, based upon availability. “U”nique Dental Services is not responsible for travel fees, or any expenses incurred by the client as a result of such cancellation.
